Can I create public-facing form and packet links for my website?

Use a URL link or an embedded button on your website to share NexHealth forms with patients.

To create a public-facing URL, any form or forms must be part of a Packet. To create packets, read our article on creating form packets.

Overview

Add Forms to your website

Generate a URL or embeddable HTML to add to your website for patients to find.

1. Navigate to Forms → Manage formsPackets tab.

Go to Forms in the left-hand sidebar, select Manage forms, then switch to the Packets tab.

2. Click on the packet’s ellipsis (...)

Click on the ellipsis (...) to the right of Edit on the packet you want to make public.

3. Select Public packet access.

Select Public packet access from the menu.

4. Copy the URL or embeddable HTML code.

Options include a URL, or an embeddable widget from HTML code.

5. Work with your web developer to add to your site.

Your web developer can embed this code or URL on your office site so patients can find forms to complete online.

When a patient completes forms via the public packet link, you must Assign & sync them to the patient from the Forms tab to ensure they are connected to the correct patient in NexHealth (forms completed via public packet links do not sync automatically). Read more in the article on form syncing.
