Overview
Create form packets
Smart form automation allows you to automatically send forms with your reminders, but you can also create packets to group forms that are commonly sent together in a manual form request. Note: Changes to packet contents do not automatically affect Smart Forms automation. If you want to update which forms are sent automatically, you must manually adjust the Smart Forms settings in the NexHealth application.
If you use the NexHealth iOS app for the iPad, packets you create here will also appear on the iPad. After updating a packet, close and reopen the iPad app to ensure the updated packet view is loaded.
Forms outside of packets will appear in alphabetical order. If you have a preferred order you would like patients to encounter their forms as they fill them out, you can name each form with a number (e.g. 1 - Patient Information, 2 - Insurance Information, etc.). This will cause the forms to be ordered numerically.
1. Navigate to Forms → Manage forms → Packets tab.
Go to Forms in the left-hand sidebar, select Manage forms, then switch to the Packets tab.
2. Click New packet.
Click New packet in the upper right of the Packets tab.
3. Name the Packet.
Give the packet a title your staff will recognize, for example, New Patient Paperwork.
4. Select the forms to add.
Add the forms to your packet by searching for the form in the Add forms to packet field. If you want to adjust the order - just click and drag your forms within the packet. You can also remove outdated forms from the packet to streamline its contents.
5. Click Save.
Click Save to complete the Packet. |

Smart Forms Behavior
Smart Forms automation is configured separately from packet contents. To update which forms are sent automatically:
Update Smart Forms Settings: Navigate to the Smart Forms configuration in the NexHealth application and make the necessary adjustments.
Understand the Separation: Changing the packet to include a different form does not alter Smart Forms automation. Manual updates to Smart Forms settings are required to reflect any changes.
Updating Form Packets
To update an existing packet:
Replace or Add Forms: Replace an existing packet’s forms or add new ones as needed. Remove outdated forms to keep the packet relevant.
Refresh the Application: After making changes, refresh the application to ensure updates are reflected. For the iPad app, close and reopen it to load the updated packet view.
