Overview
Send Forms or Packets
Smart form automation allows you to automatically send forms with your reminders, but you can also send individual forms or form packets manually.
If you use the NexHealth iOS app for the iPad, you can also send Forms from the iPad.
Forms will appear in alphabetical order. If you have a preferred order you would like patients to encounter their forms as they fill them out, you can name each form with a number (e.g. 1 - Patient Information, 2 - Insurance Information, etc.). This will cause the forms to be ordered numerically.
1. Navigate to Forms → Request forms.
Go to Forms in the left-hand sidebar, then click Request forms in the upper-right.
2. Search for and select a patient to Send to.
Use the search bar to search for a patient, then select the patient.
3. Search for and select Form(s) to send.
Use the search bar to search for forms to send, or use the quick-add buttons. The quick-add buttons are generated from your most commonly sent forms.
4. Click Send
Before clicking Send, you can also edit the expiration date by using the pencil and paper button.
You can also Customize message by toggling on message customization and typing in the personalized message.
