What do patients experience when they fill out forms with NexHealth?

To complete forms, patients will verify their identity. After completion, they’ll end on a confirmation page.

Forms patient experience

1. Patient verifies their identity.

When a patient receives a link to complete forms, either by text or email, they are first asked to verify their identity by entering their birthdate.

If the patient is accessing the form via a URL or embedded HTML on your website, they will be asked to fill in their first name, last name, phone, email, and date of birth.

Preferred language will only appear if you have translated forms.

2. Patient will click on Forms for [Patient].

Then, they are presented with a button or link showing how many forms they need to complete.

3. Patients complete the form.

Patients complete each form, with the option to go Back if needed.

4. Patient lands on confirmation page.

Finally, when they are all finished, they end on a confirmation page indicating that they are all set.
