When a patient tries to open a NexHealth form, they are asked to verify their identity by entering their date of birth. If they enter an incorrect date of birth too many times, they are locked out and can no longer access the form. This article explains how to recognize the issue and get the patient a new form request.
How to recognize the issue
When a patient has been locked out, the patient's activity feed in NexHealth will show that they failed paperwork verification. The form request itself cannot be unlocked — a new one must be sent.
How to resolve it
Open the patient's profile in NexHealth.
Locate the locked form request and click Archive to remove it.
Send the patient a new form request.
When you send the new request, let the patient know to enter their date of birth exactly as it appears in your records to avoid being locked out again.
