The best, most secure way to collect credit card information in forms is using the Payment Method component.
Do not use text fields to collect payment methods.
Overview
The Credit Card Authorization Form template
The easiest way to collect credit card information is to select the Credit Card Authorization Form template. This feature is only available to customers who are using NexHealth Payments.
1. Navigate to Forms β Manage forms.
Use the lefthand sidebar to select Forms, then select Manage forms from the upper-right.
2. Click on New form and select Build.

3. Click on the Select a Template dropdown.

4. Choose Credit Card Authorization Form.
The list is in alphabetical order.
5. Select a Sync location from the Document Type dropdown.
6. Click Save and exit.
You can now add rules to send the form to specific patients as part of their reminder sequence or add the form to existing packets.
Once a patient completes the form and the form syncs to NexHealth, the credit card information will be stored in the patient profile so you can charge the card on file when needed.
Add the Payment Method component to an existing form
You can also add the Payment Method component to any existing form to collect payment information. This feature is only available to customers who are using NexHealth Payments.
1. Open the desired form.
Navigate to Forms --> Manage forms, then locate the form you want to add the credit card field to and click to open the form.
2. Drag the Payment Method component onto the form.
Click and drag the Payment method component onto the desired location on the form.
3. Select a Sync location from the Document Type dropdown.
Be sure to update the Document Type if you want the form to sync to a new location.
4. Click Save and exit.
Once a patient completes the form and the form syncs to NexHealth, the credit card information will be stored in the patient profile so you can charge the card on file when needed.
