The Medical history form is dynamic, and it changes options for medical alerts (conditions, medications, and allergies) based on the medical alerts already in your health record system. These options are read directly from your health record system.

Overview
What NexHealth reads from Open Dental
NexHealth reads all the alerts from Problems, Allergies, and Medications.
If an office marks an alert as ‘Hidden’ NexHealth will not read these alerts.
Medications with a - in the notes will not be read in.
NexHealth will read in the alerts exactly how they appear in OpenDental (spelling & capitalization).
Any misspelled alerts must be fixed in Open Dental.
How to edit medical alerts in Open Dental
To remove any alerts from NexHealth they must be deleted from their alerts.
This Help Center article from YAPI offers useful tips.
NexHealth no longer creates long-form questions from alerts that have quotation marks around them.
Example: "Please list any medications you are currently taking" will not create a long-form question.
1. Click on Lists and select Problem / Allergies / Medications
Select Lists from the menu bar at the top of Open Dental, then select Problems, Allergies, or Medications.
2. Click +Add to add an alert.
3. Double-click on an alert and click Delete to delete an alert.
How alerts write back into Open Dental
NexHealth marks all alerts that patients marked ‘Yes’ to.
If a patient has an active alert as ‘No’ NexHealth will remove that alert.
If NexHealth hides an alert on the Medical History form and this alert is an active alert for the patient, we will leave that as an active alert in OpenDental.
If the patient adds any additional alerts, NexHealth will add these to an ‘Other’ alert we create.

If the patient responds to any of the Additional Questions they will go appear in the Problems section, use the Other Conditions (NexHealth) alert, and place the question and answer as a note for this alert.
