Collecting patient credit card information has never been easier! There are two options to collect patient payment information securely:
Credit Card Authorization form
To add the Credit Card Authorization form to your list of forms:
1. Navigate to Forms --> Manage Forms.
2. Click on New Form and select Build.

3. Click on the Select a Template dropdown to view the selection of customizable templates.
4. Choose Credit Card Authorization and click Save and exit to add it to your forms list. You can now add rules to send the form to specific patients as part of their reminder sequence or add the form to existing packets.
Once a patient completes the form and the form syncs to NexHealth, the credit card information will be stored in the patient profile so you can charge the card on file when needed.
Add the Credit Card Authorization component to an existing form
You can also add a Credit Card Authorization component to any existing form to collect payment information.
1. Navigate to Forms --> Manage Forms.
2. Locate the form you want to add the credit card component to and click to open the form.
3. Click and drag the Credit card authorization component onto the desired location on the form.
4. Click Save and exit.
Once a patient completes the form and the form syncs to NexHealth, the credit card information will be stored in the patient profile so you can charge the card on file when needed.
