Getting a doctor or staff member to sign after the patient

  1. In the form builder, add two signature fields.

  2. Label the second one (e.g., "Doctor Signature") and mark it as not required.

  3. The patient fills out the form, skips the optional field, and submits. The form syncs to the EHR as usual.

  4. Once the patient has submitted, go to their profile in NexHealth, open the form, and have the doctor add their signature. Hit Save & Sync.

Adding practice-side information to a form

If you need staff to fill in something the patient shouldn't fill in themselves:

  1. Send the form request to the patient.

  2. Before the patient opens it, pull up the form from the NexHealth dashboard and fill in the practice-specific fields and hit Save.

  3. The patient can then complete the rest.

You can also do this after the patient submits - just open the form from their profile and add whatever's needed.
