NextGen Office customers using NexHealth Forms have access to an integrated medical history form, which automatically syncs patients’ allergies, conditions, and medications to the patient health chart in NextGen Office.

The Medical history form is dynamic, and it changes options for medical alerts (conditions, medications, and allergies) based on the medical alerts already in your health record system. These options are read directly from your health record system.

Overview

What NexHealth reads from NextGen Office

  • The NexHealth Medical History Form includes search-and-select free-text questions for patients’ Allergies, Conditions and Medications.

  • The patient types in their corresponding health history, which automatically pulls matching options from NextGen Office.

  • NexHealth reads all the problems, allergies, and medications from NextGen Office.

  • NexHealth will read in the alerts exactly how they appear in NextGen Office (including spelling & capitalization).

  • Any misspelled alerts must be fixed in NextGen Office.

How alerts write back into NextGen Office

  • The patients’ Allergies, Conditions and Medications are automatically synced to the corresponding health chart fields in the patient file in NextGen Office:

    • Conditions = Problem List

    • Allergies = Allergies

    • Medications = Medication List

  • The PDF forms are synced to the Documents section of the patient file

Medical History Form FAQ

Q: Will any new locations that I add also have the Medical History Form?

A: Yes! The Medical History form is automatically generated when the Synchronizer is installed.

Q: Can I add more questions to the Medical History Form?

A: Absolutely! Our Support team is happy to help augment the form, but please keep in mind that additional questions will not sync to the health chart like Allergies, Conditions, and Medications do.
